Many serious accidents have occurred due to machines started to run when it was supposed to be completely off for repairs or maintenance. These incidents are typically caused by the energy being not released properly. Lock-out-tag-out (LOTO) that ensures that machines remain off until a designated individual releases them, helps avoid these types of accidents.

All "affected", "authorized" and "qualified" personnel must be taught about the use of these devices. These devices include, but are not only restricted to locks, chains and padlocks. Each device is unique and labeled, indicating the date and time at which it was used, the name and title of the authorized person, and the purpose of the lockout/tagout (repairs, maintenance or repairs, etc.). It must be clearly stated that the devices cannot be removed until the LOTO procedure is complete.
